WORK INDEPENDENTLY - 1-to-1 IN-HOME PATIENT CARE

Are you a dedicated individual with a heart for compassion and kindness? If you are, we want you to join our team!

• Perform home visits for patients in a home care setting as delegated by the care-managing RN
• Observe, record, and coordinate care with multi-disciplines regarding the patient's treatment and any change in condition
• Provide compassionate, patient-centered personal care including bathing/showering, washing hair, nails, continence care, etc. May also perform light housekeeping duties such as fixing a light meal, changing linen/making bed, emptying trash, etc.

• Current certification as a CNA in Connecticut REQUIRED
• Valid driver’s license
• Reliable transportation and willing to travel in New London County
• Strong communication and interpersonal skills
• Home health experience preferred
