NOW HIRING a Physical Therapist for per diem opportunities in a home care setting. You will be paid for each completed patient visit (with documentation).

Responsibilities- Perform home visits for patients in a home care setting
- Observe, record, and coordinate care with multi-disciplinary team regarding the patient's treatment and any change in condition
- Teach, supervise, and counsel the patient and family regarding home therapy procedures and other care needs as appropriate to the patient’s condition
- Current license as a Physical Therapist in Connecticut
- Valid driver’s license
- Reliable transportation and willingness to travel in New London County
- Strong communication and interpersonal skills
- Home health experience including Medicare preferred
