We are seeking a skilled and compassionate MDS Nurse to join our team at Douglas Manor Nursing and Rehabilitation Center in Windham, Connecticut. As a MDS Nurse, you will play a vital role in ensuring the highest level of care for our residents by conducting comprehensive assessments and developing individualized care plans. Responsibilities
  • Conduct thorough assessments on all residents, including physical, social, and emotional evaluations
  • Develop and implement individualized care plans in collaboration with the interdisciplinary team
  • Ensure accurate and timely completion of Minimum Data Set (MDS) assessments and care plans
  • Communicate effectively with residents, families, and healthcare professionals to ensure continuity of care
  • Participate in quality improvement initiatives to enhance the overall quality of care
  • Document resident information in accordance with regulatory requirements and company policies
Requirements
  • Current nursing license in the state of Connecticut
  • Minimum 1 year of experience as a nurse in a long-term care setting
  • Ability to work effectively in a fast-paced environment with multiple priorities
  • Strong communication and interpersonal skills
  • Ability to maintain confidentiality and discretion in handling sensitive resident information
