You will evaluate individuals in their homes, group homes, and care facilities to determine Long Term Care needs and placement options. The role requires commuting within and covering Cumberland & York County.
Essential duties include completing assessments, using standardized tools, and documenting outcomes in a HIPAA-compliant manner. Travel >50% per workweek is expected to support program delivery.
