Licensed Practical Nurse, LPN

• Provide compassionate, high-quality care to patients in their homes
• Deliver the right care at the right time and in the right place
• Build meaningful relationships with patients and families
• Travel between patient homes and assigned work locations
• Provide safe, patient-centered clinical care
• Collaborate with a supportive team of healthcare professionals

• Current Licensed Practical Nurse (LPN) license in the state of employment
• Minimum of one (1) year of experience in an acute care or comparable clinical setting
• Current CPR certification through the American Heart Association (AHA) or American Red Cross (ARC)
• Compassionate, patient-centered approach to care
• Strong communication and interpersonal skills
• Must possess a valid driver's license, maintain an acceptable driving record, OR have reliable transportation to travel between patient homes and assigned work locations
• Commitment to providing safe, high-quality patient care
