GENERAL DESCRIPTION OF DUTIES

Provides daily oversight of the OASIS Center and monitors daily, weekly, and monthly data reports. Works with the Sr. Director and Quality Assurance to ensure program goals, objectives, and contract requirements are consistently met. Ensures CRT representation at community meetings, including DSS, Journey Home, Home Care, shelter affiliates, and other groups as needed. Prepares updates for these groups and participates in monitoring budget expenditures.

SPECIFIC

DUTIES AND RESPONSIBILITIES ESSENTIAL JOB FUNCTIONS
  • Oversees volunteers and interns.
  • Schedules, organizes, and attends team meetings.
  • Works closely with Journey Home, DSS, referral sources, and medical providers that support the program.
  • Prepares timely and accurate reports for funding sources as requested by the Director.
  • Obtains all required training and certifications in evidence-based interventions used in Home Care and entitlement programs.
  • Serves as a role model for staff and clients.
  • Assesses program needs.
  • Stays informed of daily activities and identifies opportunities for program improvement.
  • Networks on behalf of CRT and OASIS with community agencies.
  • Arranges and maintains schedules, referrals, appointments, and related documentation.
  • Works closely with referral sources to strengthen services for high-risk clients.
ADDITIONAL JOB FUNCTIONS
  • Works with and assists the Director in overseeing program design, overall planning, project implementation, and client needs.
  • Supports the development of opportunities that further the mission and enhance the growth of OASIS. Works with internal CRT departments as needed to develop, plan, and create new program enhancements.
  • Designs, teaches, and implements approaches for delivering a range of supports and interventions to clients.
  • Completes entitlement assessments for clients.
  • Provides timely, professional updates to all parties involved in client care.
  • Ensures all clients are accurately entered into the STS system with required paperwork and FNPI service goals.
  • Participates in audits and remains audit-ready at all times.
  • Ensures clients’ daily basic needs are met.
  • Performs all other duties as assigned.
MINIMUM TRAINING AND EXPERIENCE

Education:

CHW certification , health care administration, business administration, or a closely related field. Relevant experience may be substituted for academic credentials. Required or Preferred

Minimum Years of

Experience:

Minimum of two years’ experience in the Case management/Community Health coordination services, particularly in Homelessness , substance use, and/or HIV services. Required or Preferred

Active Certification in Good Standing:
Certified as a Peer Specialist, Recovery Coach, Community Health Worker, or Addiction Counselor, or willing to obtain certification. Required or Preferred

Demonstrated

Skills:

Excellent interpersonal, oral and written communication, public speaking, problem-solving, and decision-making skills. Strong computer skills and the ability to work effectively with staff and participants from a wide variety of backgrounds.

Knowledge of: JEDI principles—justice, equity, diversity, and inclusion. Experience working with all types of people and program administrators a strong understanding homeless programs.

Bilingual:
Preferred

Active driver’s license in good standing is required upon hire and throughout employment.
