Advanced Practice Clinician (Nurse Practitioner / Physician Assistant)

Location:

Hartford County, CT

Schedule:

Monday through Friday, 8:00 AM to 5:00 PM

Duration: 1 year Contract with potential for permanent hire

Hours:

40 hours per week

Setting:
Senior Living / Long-Term Care Facilities

Travel Required:

Yes, between 2 facilities during the workday

Overview

We are seeking an experienced Advanced Practice Clinician (Nurse Practitioner or Physician Assistant) to provide comprehensive, patient-centered care for high-risk individuals residing in senior living and long-term care settings. This role is part of an interdisciplinary care team focused on delivering longitudinal, coordinated care that improves health outcomes and enhances quality of life.

This position offers a predictable weekday schedule with no on-call responsibilities, weekends, or holidays, supporting a strong work-life balance.

Responsibilities
  • Conduct comprehensive health assessments and evaluations.
  • Diagnose, treat, and manage acute and chronic medical conditions.
  • Address behavioral health concerns as appropriate.
  • Maintain accurate and timely clinical documentation, including ICD-10 diagnosis coding.
  • Collaborate with physicians, nurses, social workers, and other members of the interdisciplinary care team.
  • Participate in advanced care planning and goals-of-care discussions with patients and families.
  • Provide patient and caregiver education regarding disease management, treatment plans, and preventive care.
  • Develop and implement individualized care plans to address complex healthcare needs.
  • Monitor patient progress and adjust treatment plans as necessary.
Required Qualifications Nurse Practitioner
  • Active and unrestricted Nurse Practitioner license in the state of practice or ability to obtain prior to start date.
  • Graduate of an accredited Master of Science in Nursing (MSN) or Doctor of Nursing Practice (DNP) program.
  • Current national board certification as one of the following:
    • Family Nurse Practitioner (FNP)
    • Adult Nurse Practitioner (ANP)
    • Gerontology Nurse Practitioner (GNP)
    • Adult-Gerontology Acute Care Nurse Practitioner (AGACNP)
Physician Assistant
  • Active and unrestricted Physician Assistant license in the state of practice or ability to obtain prior to start date.
  • Graduate of an accredited Physician Assistant program.
  • Current NCCPA certification.
Additional Requirements
  • Current DEA registration and prescriptive authority.
  • Ability to obtain a collaborative practice agreement, if required by state regulations.
  • Reliable transportation for travel between patient care locations.
  • Ability to perform physical aspects of the role, including:
    • Lifting and carrying up to 30 pounds.
    • Navigating stairs and varying facility environments.
    • Sitting, standing, bending, and kneeling to conduct patient assessments.
  • Compliance with applicable vaccination, health screening, and credentialing requirements.
  • Ability to successfully complete required pre-employment screenings.
Preferred Qualifications
  • At least 1 year of clinical practice experience; long-term care, skilled nursing, or senior living experience preferred.
  • Experience caring for patients with complex medical, behavioral, social, and functional needs.
  • Knowledge of geriatric medicine and chronic disease management.
  • Experience conducting advanced illness, palliative care, and end-of-life discussions.
  • Strong computer proficiency and experience documenting in electronic medical record (EMR/EHR) systems.
  • Excellent communication, collaboration, and patient education skills.
