21st Century Home Health Services is seeking a Registered Nurse Case Manager, Level 2, to oversee home visits in Windsor and surrounding areas. You will create, implement, and monitor plans of care to promote wellness and prevent re-hospitalization for patients in their homes.
You will lead two nurses, conduct Start of Care, Evaluations, Resumption of Care and discharge planning, and educate patients and families on wound care, CHF management, medications, and home safety.
