Pelican Pointe Healthcare & Rehabilitation in Windsor, CO is a post‑acute care facility offering long‑term care, short‑term care, and geriatric outpatient services. We provide a stable and collaborative in‑house therapy team that supports our geriatric clientele on their journey to recovery.
We are currently seeking a Full‑time Occupational Therapist for our in‑house rehabilitation program.

Qualifications- Licensed Occupational Therapist
- Open to applicants of all experience levels
- Effectively screen and evaluate patients with cognitive and/or ADL and physical functioning disorders and develop an appropriate plan of care following all regulatory and clinical practice standards.
- Utilize standardized assessments and evidence‑based practice to support clinical interventions.
- Provide rehabilitative, skilled and medically necessary treatment intervention to patients with cognitive and/or ADL and physical functioning disorders per regulatory and clinical practice requirements.
- Administer various procedures as part of the rehabilitation plan, including ADL retraining; therapeutic activities; therapeutic exercises; modalities; use of supportive and assistive devices; etc.
- Assess patient’s progress and adjust treatments accordingly in the clinical record per regulatory and clinical practice requirements.
- Evaluate treatment outcomes for effectiveness. Modify services in relation to patient evaluation and refer as needed.
- Provide consultation and counseling to patients, families, caregivers and other service providers related to physical disorders.
- Effectively communicate with supervisor and other health team members regarding patient progress, barriers, and treatment plans.
- Provide a comprehensive discharge summary of services provided per regulatory and clinical requirements.
