Join a collaborative and well-established Family Medicine team at Hartford Health Care as a Nurse Practitioner or Physician Assistant in Primary Care. This supportive practice includes 4 physicians and 1 APP, an embedded behavioral health provider, and a clinical pharmacist, all working together to deliver comprehensive, patient-centered care. The Windsor Elite team is known for its outstanding culture, efficient workflows, and strong collaboration between providers, nursing, medical assistants, and front office staff.

Nurse Practitioners And Physician Assistants Role ScopeUnder established guidelines and in collaboration and/or supervision with the practice physician(s), Nurse Practitioners and Physician Assistants perform routine physical examinations, and formulate medical, educational, and psychosocial plans to treat patients.
In consultation with the physician(s) and care team members, Nurse Practitioners and Physician Assistants develop plans for further evaluation of health needs, e.g., referral to other health team members or specialists.
Nurse Practitioners or Physician Assistants provide appropriate treatment for problems within the scope of the provider’s practice as designated by protocols and identify problems outside of their scope of practice and refer appropriately to physician.
Qualifications- Licensure:
Unrestricted State of Connecticut Medical License (or eligible) - Training: APRN (Family Nurse Practitioner) or PA
- Experience:
1+ years’ experience in a primary care or relevant outpatient setting
