Advanced Practitioners - Primary Care
Job Details- Join a collaborative and well‑established Family Medicine team as a Nurse Practitioner (FNP) or Physician Assistant (PA) in a 100% outpatient Primary Care setting
- Supportive practice includes 4 physicians and 1 APP, an embedded behavioral health provider, and a clinical pharmacist working together to deliver comprehensive, patient‑centered care
- Full‑time position with weekdays only schedule - no call required
- Perform routine physical examinations and formulate medical, educational, and psychosocial plans to treat patients
- Develop plans for further evaluation of health needs, including referrals to other health team members or specialists
- Provide appropriate treatment for problems within the scope of practice and identify issues requiring physician referral
- Family Medicine panel serving patients from birth and older
- Work within an outstanding team culture known for efficient workflows and strong collaboration between providers, nursing, medical assistants, and front office staff
- Unrestricted State of Connecticut Medical License required (or eligible)
- APRN (Family Nurse Practitioner) or PA training required
- Minimum 1+ years of experience in a primary care or relevant outpatient setting required
- Access to dedicated Nurse Practitioner and Physician Assistant support through the Office of Advanced Practice
- Established mentorship and orientation programs available for newly hired providers
