Premier Home Health Providers Inc. is seeking a per diem Licensed Practical/Vocational Nurse to provide direct patient care in home settings.
You will implement care plans, educate families, and collaborate with OT, PT, HHA, and the RN to ensure safe, high-quality patient care. Applicants must be a graduate of an accredited LPN/LVN program with an active license in California, 3+ years of nursing experience (home health or med/surg preferred), and CPR certified.
