Nurse Practitioner - Family Practice

Job Overview

UW Health is seeking a Nurse Practitioner – Family Practice to work primarily in Windsor, Wisconsin on a full‑time, Monday‑Friday schedule (8:00AM – 5:00PM). No holidays or call.

• Perform annual physical exams and manage health maintenance for a family medicine population.
• Care for the acute and chronic needs of patients from newborn to elderly.
• Conduct telemedicine visits via video and telephone.
• Execute routine clinical procedures independently, including wart removal, pap smears, IUD removal, IUD/Nexplanon placement, and skin lesion removal.

• Must be a licensed Nurse Practitioner or Physician Assistant in the State of Wisconsin.
• For Nurse Practitioners:
  Master’s or Doctoral degree from an accredited nursing program; active RN and APRN certifications; active board certification in Family Practice.
• For Physician Assistants:
  Bachelor’s or Master’s from an accredited PA program; active PA license; active board certification.
• Preferred: minimum two (2) years of relevant clinical experience.
• DEA license or ability to obtain one prior to or upon hire.

• State of Wisconsin NP or PA licensure.
• Prescriptive authority for the NP or PA license.
• DEA license or eligibility to obtain one.
