This full-time position supports patients in Glen Ellyn and surrounding areas and is eligible for a $20,000 total bonus potential, including a $10,000 first year success bonus and an additional $10,000 sign-on bonus paid within the first 18 months of employment. To receive these bonuses, you must be in good standing and meet performance expectations, and be in the role and status for which you were hired.

• Collaborate to provide management and delivery of patient care plans as part of a team-based home health environment.
• Support patients and participate in scheduling and care coordination as part of Residential’s care team.
• Engage in ongoing professional development and utilization of clinical educators, preceptors, and supervisors to mentor and guide.

• Bachelor’s degree or Master’s degree in Physical Therapy from an accredited college required
• Licensed Physical Therapist issued by the state in which you will be working
• Minimum of one year healthcare experience
• Current driver’s license and ability to spend ~20% of your day driving to/from patient locations
• Ability to consistently meet critical deadlines for charting and self-manage time and scheduling
• Demonstrated experience with tablets, mobile phones and EMR software
