Position: Registered Nurse (RN) - Home Health - $75K-100K per year

Job Overview

Graham Healthcare Group is seeking a Registered Nurse (RN) Home Health to provide care in Glen Ellyn, Illinois and surrounding areas. The position is full‑time, 40hours per week, with an 8‑hour shift. The ideal candidate will manage and deliver patient care plans in a team‑based environment.

Responsibilities
  • Lead and collaborate as a home‑health registered nurse case manager to manage and deliver patient care plans.
  • Manage patient care documentation and meet critical charting deadlines.
  • Coordinate care team activities and maintain communication with patients, families, and healthcare providers.
  • Maintain up‑to‑date documentation and comply with billing, clinical, and regulatory requirements.
Qualifications
  • RN license in the state of employment.
  • Graduate from an approved school of practical nursing.
  • Minimum one year of RN experience and the clinical competence to deliver quality patient care.
  • Current driver’s license and ability to drive approximately 20% of work hours to patient locations.
  • Commitment to consistently meet critical charting deadlines and self‑manage time and schedule.
  • Demonstrated experience using tablets, mobile phones, and EMR software.
  • Successful completion of a drug screen (medical and recreational marijuana) as part of the background process.
  • Legally authorized to work in the United States.
