Infection Control and Assessment Nurse
1.0 FTE, 80 hrs pay period
Days, 7:30am-5pm
Weekends:
Every 4th weekend
Holidays: 1 major, 1 minor
Position Overview: The Infection Control (IC) and Assessment Nurse supports long‑term care residents by facilitating appropriate assessment, evaluation and action needed to improve quality of care according to evidence‑based practices. This individual identifies risks and incorporates appropriate approaches, documentation and monitoring of processes and care. The IC and Assessment Nurse works closely with each unit team at Lake Winona Manor as a mentor and facilitator for the delivery and execution of the care plan process.
Responsibilities- Perform thorough and timely admission, quarterly, and other MDS‑related assessments as required by the State of Minnesota.
- Evaluate residents holistically to identify risks, strengths, and needs; collaborate to develop individual care plans, especially on admission and during wound and infection care.
- Participate on the Winona Health Committee as a representative of senior services.
- Analyze infections, treatments, and drive mitigation and prevention strategies.
- Track resident and caregiver infections, organisms, and develop reports for the care team, including the Quality Assurance & Improvement Committee.
- Serve as a member of the Quality Assurance and Improvement Committee and use LEAN methodology to make daily improvements.
- Provide complete documentation for open areas/wounds of residents on a weekly/prn basis.
- Document wound‑care effectiveness, adjust treatment, and collaborate with Part B Wound Care services in the facility.
- Ensure the plan of care is visible and adhered to by the interdisciplinary team; conduct audits on care‑plan adherence.
- Document ongoing and acute issues appropriately, completely and timely.
- Support staff in identifying significant changes in resident needs and services.
- Identify gaps and support needed for successful discharges of short‑term residents.
- Understand the nursing practice process and stay current on nursing theory, research, medications and legal issues related to resident‑based services.
- Assist with other nursing duties and performance‑improvement projects as assigned (e.g., vaccine administration, COVID‑19 outbreak testing, Kaizens/CIP event teams).
- Provide care and supervision of caregivers and residents at the level of licensure.
- Deliver nursing services to residents in accordance with facility policy and professional standards of care.
Required:
- 2 years of RN experience
- CPR certification
- MN RN license
- Complete Infection Control course within 6 months of hire (provided by WH)
- Complete Wound Care training within 6 months of hire (provided by WH)
Preferred:
- Basic computer skills – Word, Excel, Outlook, EMR
- Non‑violent crisis intervention and crucial conversation training within the first year.
