Position Overview

A Home Health Aide / Hospice Aide is a certified health care worker who assists professional personnel in the personal cares of the patient by providing necessary and desired services. The duties are planned and assigned by a professional member of the health care team to meet the needs of the patient and family.

Essential Duties & Responsibilities
  • Provide a sense of security for the patient and perform personal cares while respecting dignity, comfort and privacy.
  • Provide physical and emotional support to patient and family unit.
  • Adapt to changes in scheduling to meet patient needs.
  • Accept direction positively, eliminate assumptions, demonstrate effective listening and follow instructions.
  • Adhere to strict patient confidentiality standards as mandated by HIPAA.
  • Utilize supplies safely, proficiently and orderly.
  • Document tasks accurately and timely in the patient’s clinical record and contribute to continuous process improvement activities.
  • Attend continuing education and staff meetings as scheduled.
Essential Skills and Experience
  • Graduate of a state approved Certified Nursing Assistant Program.
  • Valid Driver’s License.
  • CPR Certification.
  • Certified Nursing Assistant.
  • Approved by both the MN and WI Nursing Assistant Registry.
  • Basic computer skills.
Nonessential Skills and Experience
  • Hospice certification desirable.
