Justice‑Involved Case Manager
We are seeking motivated, mission‑driven Case Managers to support individuals preparing for release and successful reentry into the community.
This role focuses on serving individuals connected to Arizona correctional facilities, probation/parole systems, and reentry programs, ensuring continuity of care, compliance with court/program requirements, and long‑term stability.
The ideal candidate understands that reentry is not just about release—it’s about structure, accountability, and support across multiple systems.
This position plays a direct role in:
- Reducing recidivism
- Improving treatment engagement
- Stabilizing housing, employment, and recovery outcomes
This aligns with our internal program model where case managers provide care coordination, service linkage, and compliance support for justice‑involved populations.
Key Responsibilities Justice System Coordination- Maintain communication with correctional facilities, probation, parole, and courts
- Support individuals in understanding and complying with legal obligations
- Coordinate reentry services prior to and immediately following release
- Conduct assessments, identify needs, and develop individualized service plans
- Connect clients to:
- Housing
- Employment
- Behavioral health and SUD treatment
- Medical and community resources
- Provide ongoing follow‑up and progress monitoring
These responsibilities align with our structured case management model including assessment, planning, coordination, and advocacy for justice‑involved clients.
Reentry & Stability Support- Assist clients transitioning from incarceration into the community
- Support routines, accountability plans, and service engagement
- Address barriers such as transportation, documentation, benefits, etc.
- Maintain accurate, timely documentation
- Ensure compliance with HIPAA, 42 CFR Part 2, ADHS, and AHCCCS standards
- Participate in case staffing, team meetings, and reporting
- Travel within assigned regions near correctional facilities
- Build relationships with reentry coordinators and facility staff
- Represent the organization in justice‑system collaboration efforts
- Bachelor's degree in:
- Social Work
- Behavioral Health
- Criminal Justice
- Psychology
- Related field or equivalent experience
- Experience working with:
- Justice‑involved individuals
- Behavioral health and/or substance use populations
- Strong knowledge of:
- Court systems, probation, and parole processes
- Community resources and reentry services
- Ability to:
- Maintain professional boundaries
- Work independently in field‑based environments
- Manage documentation and multiple caseloads
- Valid Arizona driver's license and reliable transportation
- Current hands‑on CPR/First Aid certification
- Current Fingerprint Clearance Card (FPCC)
Ability to pass background screening and obtain required badge/access clearance for entry into the prison.
- Ability to complete and maintain annual TB testing
- Experience inside or working with:
- Prisons, jails, or reentry programs
- Probation or parole populations
- Bilingual (English/Spanish)
- Training in:
- Trauma‑informed care
- Motivational Interviewing
- Solution‑focused practices
- Community‑based, facility‑based, and office‑based work
- Travel required within assigned Arizona region
- Regular interaction with justice system partners and high‑need populations
- Direct impact on reducing recidivism and improving community outcomes
