Health Education and Aging Resource Team – Masters Level or Licensed Clinician

Join Victorian Senior Care as a Health Education and Aging Resource Team (HEART) professional. This role focuses on education and training for staff in assisted living facilities and related community agencies.

• Educate long‑term care facility staff through in‑service training classes.
• Provide consultation on behavior management, crisis planning and other intervention techniques.
• Coordinate care with psychiatrists and other mental health providers to families and LTC facilities.
• Offer case consultations as requested and maintain documentation of all services.
• Network with LTC facilities and community agencies (DSS, Ombudsman) and other stakeholders.
• Maintain compliance with regulatory requirements and submit monthly reports for billing.
• Assist with referrals to community‑based services and provide resource information.
• Travel to agencies to provide education, training, and market program services.
• Attend required meetings and support team goals.
• Perform other duties as required.

• A Master’s degree in a human services field or a licensed clinician with appropriate credentials.
• Minimum one year of experience with the geriatric population, as required by state regulations.
• Minimum one year of experience working with individuals in the mental health and geriatric fields.
• No substantiated findings on the NC Personnel Registry or Health Care Program.
• Valid NC Driver’s License with a clean SBI/DMV report.
• Minimum 8 years of driving experience; ability to operate a vehicle in varying weather conditions.
• Physical ability to lift and carry up to 25 lbs, repeat lifting, and endure walking and standing for extended periods.
• Ability to work indoors and in the community, including exposure to various road and weather conditions.
