Pay Range $41.10 - $61.65

Job Summary: Supports the needs of people in the region by design, management, and implementation of Heart and Vascular Subspeciality programs that standardizes care and facilitates continuous improvement to achieve quality outcomes within the Service Line.

Major Responsibilities
  • Develops and manages programs to support quality, efficiency, and national recognition for excellence in Subspecialty Heart and Vascular care.
  • Serves as a primary point of entry for providers and patients requiring extensive coordination of care and helps the patient overcome obstacles to timely diagnosis and treatment.
  • Educates and collaborates with multidisciplinary team to plan and implement clinical research related care to participating research subjects and families.
  • Responsible for patient and family from the time of diagnosis through treatment and continuing through the outpatient clinic.
  • Promotes knowledge of disease process for the patient, family, and care providers.
  • Enhances the Service Line by developing, implementing and directing the Subspecialty program, services, systems and resources.
  • Contributes to information network formation by developing and maintaining linkages internally and with regional hospitals, clinics, and providers.
  • Manages individual workload and expertise.
  • Promotes the Program by developing and implementing plans in conjunction with Marketing Department.
  • Supports program operations by completing other duties as assigned or required.
Education

Required:

Bachelor's degree in Nursing.

Minimum

Job Requirements
  • Bachelor's degree in Nursing required.
Certification / License
  • Current licensure as Registered Nurse with applicable state Board of Nursing or compact state as defined by applicable state Board of Nursing.
  • BLS certification required.
  • ACLS and certification in relevant clinical area preferred.
Work Experience
  • Five years' Heart and Vascular or Heart and Vascular related experience required.
Knowledge / Skills / Abilities
  • Ability to provide direct and indirect patient care.
  • Excellent interpersonal skills; solid oral, written and listening communication skills.
  • Demonstrated organizational skills.
  • Basic computer skills.
Physical Requirements And Working Conditions
  • Contact with patients and family members under wide variety of circumstances.
  • Possible exposure to infectious and contagious diseases.
  • Potential exposure to radiation hazards.
  • Exposure to housekeeping/cleaning agents/chemicals.
  • Subject to many interruptions.
Preferred

Job Requirements SUBSPECIALTY PROGRAMS Structural Heart
  • Experience:

    Background in cardiology, case management and structural heart defects and disease preferred.
  • Essential Functions:

    Primary focus on the Structural Heart patient population and program.
Cardiothoracic Surgery
  • Experience:

    Background in cardiothoracic surgery, case management and patient triage preferred.
  • Essential Functions:

    Primary focus on the Cardiothoracic surgery patient population and program.
Advanced Heart Failure
  • Experience:

    Background in advanced heart failure, case management and advanced cardiovascular therapies preferred.
  • Essential Functions:

    Primary focus on the Advanced Heart Failure patient population and program.
