- Guides patients through the healthcare system, establishing contact with patients, family members, and/or caregivers. Directs patients to health care services in the organization and in the community for timely diagnosis, treatment and survivorship. Identify and address barriers to care that might prevent timely and appropriate treatment. Assist the patient and physicians in the transition from active treatment to continuing healthcare.
- Utilize nursing process to provide patient/family centered care focusing on physical, emotional, spiritual, cultural, religious, and environmental needs. Includes assessment, development of nursing diagnoses, outcome identification, planning, implementation of interventions, coordination of care, health teaching and promotion, and evaluation of goals and interventions aligned with WFBMC policies.
- Coordinate delivery of patient care services through care management principles. Identify clinical priorities and take action to improve patient outcomes. Act as patient advocate across the continuum of care and in the community.
- Document according to departmental standards. Collect and maintain up-to-date program data and documentation. Deliver reports demonstrating outcomes and performance improvement activities; adhere to productivity benchmarks.
- Implement outreach to establish and maintain positive relationships with community agencies; serve as community liaison and educator in underserved communities.
- Participate in a professional practice model with governance, standards of care, peer review, leadership development, teamwork, autonomous practice, and accountability.
- Communicate effectively to share information across the continuum of care and as a member of the healthcare team.
- Evaluate and contribute to quality improvement of the Navigator Program and track activity per program standards.
- Collaborate with community-based service providers to address community needs for services and education on prevention, early detection, medications, and first aid.
- Provide age/developmentally appropriate patient care according to Age-Specific guidelines for the served age groups.
- Graduate from an accredited school of nursing; BSN required.
- Minimum two years' experience as a registered nurse in the specialty required.
- Current licensure to practice as Registered Nurse in North Carolina.
- Certification as an RN preferred.
- BCLS required.
- Problem solving, decision making, and critical thinking
- Ability to work in a self-directed environment and with leading teams
- Ability to work in a fast-paced environment with multiple interruptions
- Expertise in MS Office
- Ability to prepare program reports efficiently
- Bilingual preference with fluency in Spanish desirable
