Carolina Caring is looking for a compassionate and skilled Nurse Practitioner to join our House Calls and Palliative Medicine Program in Davie County
. In this role, you will travel to patients’ homes, providing personalized, patient-centered care while supporting their comfort and quality of life.

• A minimum of two years in a Primary Care setting.
• Gerontology experience or certification preferred.
• Experience with CMS Chronic Care Management Services preferred.

• Graduate of an accredited school of nursing with a Master’s degree in nursing and appropriate advanced training required for licensure as a registered nurse practitioner.
• Current, unrestricted North Carolina Nurse Practitioner license.
• Must maintain CPR certification.
• Must have active DEA certificate.
