Job Summary

The Licensed Practical Nurse (LPN) administers nursing care in a skilled nursing community under the supervision of a registered nurse or another medical supervisor and operates under the direction of physician orders. The role provides basic medical care including assessing resident conditions and monitoring for changes, wound care, administering medication, resident and caregiver education, and obtaining specimens per physician orders. The LPN ensures the health, comfort and safety of patients by assisting with bathing, feeding and dressing.

Job Functions
  • Provides direct resident nursing care and supervises medication aides, medication techs and CNAs within their unit in accordance with established plans.
  • Acts in the capacity of a supervisor in a modified primary care setting.
  • Complies with evaluation, treatment and documentation of residents within their assignment.
  • Maintains ongoing communications with physicians concerning resident care, including obtaining and reporting lab results, x-ray results and physical assessment abnormalities.
  • Completes required documentation in an accurate and timely manner.
  • Confirms residents have received their scheduled showers and that ADL care is provided as expected.
  • Facilitates problem solving and open communications with the nursing staff.
  • Ensures effective interactions with patients and families.
  • Ensures compliance with resident rights policies and works with families and other staff to resolve grievances.
  • Acts in compliance with company regulatory and professional standards and guidelines.
  • Accesses continuing education opportunities appropriate to discipline and responsibilities to maintain clinical competency.
  • Adheres to facility policies and procedures and participates in facility quality improvement and safety programs.
  • Participates in or provides in-service education sessions.
  • Supports company culture and philosophy.
  • Performs other duties as assigned.
  • Completes annual compliance and HIPAA training and exhibits behavior as set forth by the Code of Compliance in the performance of their duties.
  • Adheres to company policies and procedures regarding the compliance program and reports concerns in a timely manner.
  • Follows all local, state and federal regulations as they pertain to the position.
  • Adheres to company policies and procedures concerning privacy and security in compliance with HIPAA regulations.
  • Participates in the nurse on-call schedule per policy.

  • Sensitive to changing medical needs and the nature of staff responses, taking appropriate actions regardless of whether needs have been reported by others.
  • Ability to model person-centered practices and provide direction to team members as needed.

Education and Experience
  • Graduate of an accredited Licensed Practical Nurse educational program.
  • Licensed Practical Nurse license in good standing with the North Carolina Board of Nursing.
  • CPR certified or the ability to become CPR certified.
Physical Requirements
  • Capable of standing and walking short distances for extended periods of time.
  • Ability to lift, carry, push and/or pull objects more than 50 pounds.
  • Stoop and bend at the knees, twisting from side to side at the waist.
  • Reach above, below and at shoulder level.
Work Environment

Works in a clean, well-lit facility. Possible work hazards include blood borne pathogens, moving parts, muscular pains and strong offensive odors.
