Licensed Practical Nurse

Overview

Care Partners seeks a compassionate Licensed Practical Nurse to deliver skilled nursing care to patients in their homes. In this field-based role, you will perform assessments, administer medications, provide wound and catheter care, and monitor patient progress under RN and physician direction. You’ll document in our EMR, educate patients and families, and collaborate closely with the interdisciplinary team to support safety, recovery, and independence.
