• Pay Range: 2034.00 - 2135.70
• Contract Length: 13 weeks

• Conduct comprehensive assessments of home health patients, identifying physical, psychosocial, and environmental needs.
• Develop, implement, and evaluate individualized Plans of Care to ensure quality, continuity, and patient safety.
• Gather and incorporate data on patient factors influencing health status into the Plan of Care.
• Maintain accurate and up‑to‑date patient records, documenting all visits, contacts, and care plans.
• Adhere to infection control protocols to ensure safety for patients and staff.
• Educate patients and families on care procedures, medication management, and safety practices.
• Supervise and direct home health aides, LVNs, and STNAs to ensure quality service delivery.
• Assess and manage pain using appropriate techniques, educating patients and families.
• Coordinate referrals to community agencies and support services as needed.
• Monitor cases to ensure compliance with payer requirements.
• Provide on‑call coverage and urgent care services as assigned.
• Communicate effectively with patients, families, healthcare team members, and providers.
• Attend interdisciplinary team meetings and contribute to patient care planning.
• Perform additional duties as assigned.
