Job Overview
A Registered Nurse (RN) will provide oversight and general nursing care on an Assertive Community Treatment Team (ACTT) in a regular full‑time position. The role supports individuals with intellectual, developmental disabilities, mental health, and substance use challenges in the community, delivering integrated crisis response, stabilization, and prevention interventions.
Physical Requirements- Lift 20lbs.; lift minimum 40lbs.; pull minimum 25lbs.; squat, kneel, crawl, crouch, climb, and stoop;
- Use hands to finger, handle, or feel objects, tools, or controls;
- Regularly stand and walk;
- Proficiency in CPR (floor level, hands, knees, bending, standing, lifting);
- Vision: close, distance, and peripheral;
- Must be able to talk and hear.
- Follow supports and services indicated in the recipient’s Crisis Plan, part of all Person‑Centered Plans.
- Participate as a member of the interdisciplinary team and attend Person Centered Plan (PCP) meetings.
- Develop plans as requested.
- Monitor consumers and take appropriate action for side‑effects and adverse reactions to medications.
- Assist team members with assessing learning needs of consumers and carry out teaching as indicated.
- Active RN license (unencumbered).
- Bachelor of Science in Nursing preferred.
- Preferred: at least one year of nursing experience; behavioral‑health nursing experience preferred.
- Valid driver’s license and personal vehicle insurance (condition of employment).
- Maintain proficiency in company‑sponsored training, certifications, and privileging as required.
- Background check: criminal history, registries (OIG exclusions database, Child Abuse Registry, Offenders Against Individuals with Developmental Disabilities).
- Drug testing.
- Education verification and other credentialing as required.
- Proof of employment history or references (if required).
