We’re looking for an experienced, patient-oriented nursing professional to join our team as an RN Patient Navigator at Atrium Health Wake Forest Baptist!
Position Highlights:- Schedule: 40 hours per week, Monday-Friday with rotating call schedule
- Location: Wake Forest Baptist Medical Center with possible travel to other Wake market locations

Guides patients through the healthcare system, establishing contact with patients, family members, and/or caregiver. Understands the clinical pathways established by the clinical specialty and directs patients to health care services in the organization and in the community for timely diagnosis, treatment and survivorship. Actively identifies and addresses barriers to care that might keep the patient from receiving timely and appropriate treatment for specific diagnosis.
Assists the patient and physicians in the transition from active treatment to continuing healthcare.
- Graduate from an accredited school of nursing, BSN required.
- Minimum two years’ experience as a registered nurse in the specialty required.
- 3-5 years of nursing experience strongly preferred
- Current licensure to practice as Registered Nurse in North Carolina.
- Certification as an RN preferred. BCLS required.
