This field-based role enables associates to primarily operate in the field, traveling to client sites or designated locations as their role requires, with occasional office attendance for meetings or training. This approach ensures flexibility, responsiveness to client needs, and direct, hands-on engagement.
The Peer Support Specialist is responsible for engaging with community partners and builds strategy with local stakeholders; providing education, strengthening relationships, and helping resolve case or broader systems issues. Supports care coordination teams and connect members and caregivers with those who can provide insight and expertise to assist in overcoming challenges.
How you will make an impact:
- Identify opportunities for engagement of members and their caregivers and families to share with care management team.
- Partners with peer support services in the community to create referral pathways and links members to appropriate community peer support services.
- Support care manager as they advocate for foster parents and other caregivers, addressing their needs or challenges with various agencies and systems.
- Develop strategy for peer support programs.
Minimum requirements:
- Requires a HS diploma or equivalent and a minimum 2 years of related experience; or any combination of education and experience, which would provide an equivalent background.
Preferred Skills, Capabilities and Experiences:
- Experience working with foster youth strongly preferred.
- Skilled in providing mentorship, advocacy, guidance, and peer support to help youth navigate challenges and achieve personal goals strongly preferred.
