Registered Nurse, RN

We are hiring RNs for Full-Time Home Health - Admissions/Visits.

The Registered Nurse in Home Health provides and directs nursing care to patients in their homes, as prescribed by physicians and in compliance with laws, regulations, and agency policies. The RN coordinates the overall care plan with other healthcare professionals and ensures continuity of care by sharing information with physicians, agency staff, patients, families, and community resources. All services are provided within a Point of Care setting.

• Provide and coordinate nursing care for patients at home, following physician prescriptions and legal requirements.
• Conduct initial and comprehensive nursing evaluations, develop care plans within state guidelines, and document accurately and timely in collaboration with physicians.
• Perform assessments at designated times and develop appropriate care plans.
• Ensure patients meet home health eligibility and medical necessity criteria as defined by payers.

License Requirements:

• Current RN licensure in the state of practice
• Current CPR certification
• Valid driver’s license, vehicle insurance, and access to reliable transportation or public transit
