Licensed Practical Nurse

Vital Caring is seeking a Licensed Practical Nurse to deliver compassionate home health and hospice care. In this field-based role, you'll provide hands-on nursing, including medication administration, wound care, vital signs, and symptom management under RN and physician direction. You'll educate patients and families, document in an EMR, and collaborate closely with an interdisciplinary team. Responsibilities

• Provide skilled nursing care to home health and hospice patients under RN and physician direction
• Administer medications and treatments, monitoring patient responses and reporting changes in condition
• Perform wound care, vital signs, and clinical assessments in the patient's home
• Educate patients and families on disease management, medications, and home care procedures
• Document care accurately in electronic health records and follow care plans and regulatory standards
• Coordinate with RNs, therapists, aides, and physicians to support individualized care plans
• Participate in case conferences and communicate regularly with the interdisciplinary care team
• Support patient comfort, safety, and dignity, including end-of-life and symptom management
• Reinforce infection control and safety protocols in the home setting
• Contribute to a culture of compassion, integrity, and clinical excellence
