Youth Transition Specialist

Lutheran Services Florida is seeking a Youth Transition Specialist to support adolescents and young adults as they move from foster care or youth services to independent living. This role conducts assessments, develops individualized transition and life-skills plans, and connects youth to housing, education, employment, healthcare, and community resources. The specialist provides coaching on budgeting, job readiness, and daily living skills while coordinating with case managers, schools, and service providers.

Documentation, goal tracking, and crisis support are essential. This position aligns with LSF's mission-driven, compassionate culture and offers growth in social and behavioral health services.

Responsibilities

• Conduct assessments and develop individualized transition and life-skills plans for youth
• Provide one-on-one coaching on employment readiness, budgeting, daily living, and education planning
• Coordinate services with case managers, schools, employers, and community agencies
• Link youth to housing, healthcare, education, and vocational resources
• Maintain accurate, timely documentation and progress notes in agency systems
• Monitor goals, track outcomes, and adjust plans based on youth needs
• Provide crisis intervention and safety planning as needed
• Facilitate small groups or workshops on independent living skills
• Advocate for youth needs within systems such as child welfare, education, and behavioral health
• Participate in team meetings, trainings, and community outreach to strengthen services
