CASE MANAGER

Lutheran Services Florida is seeking a compassionate Family Services Case Manager to support children and families through coordinated, community-based services. In this role, you'll manage a caseload, complete assessments, develop service plans, and connect families to child welfare, mental health, substance abuse, and educational resources. You'll collaborate with a diverse, supportive team dedicated to positive community impact, while maintaining timely documentation and ensuring compliance with agency and regulatory standards in a hybrid office and field environment.

Responsibilities

• Manage family caseloads, conducting intake, assessment, and ongoing service planning
• Coordinate referrals to community resources in child welfare, mental health, substance abuse, and education
• Monitor family progress, document case notes, and maintain accurate, timely records in case management systems
• Collaborate with internal programs and external partners to ensure integrated, holistic support
• Advocate for client needs while promoting family stability, safety, and self-sufficiency
• Facilitate family meetings, home visits, and crisis interventions as needed
• Ensure compliance with agency policies, contracts, and regulatory standards
• Collect and report outcome data to support program evaluation and improvement
• Participate in team meetings, case staffing, and professional development activities
• Educate families on available services, rights, and responsibilities
