Primary Care PhysicianWe’re looking for dedicated primary care providers who are seeking purpose and personal opportunity. Our PCPs play a pivotal role in delivering quality, value‑based care to up to 450 patients.
EssentialJob Duties / Responsibilities
- The PCP independently provides care for patients with acute and chronic illnesses encountered in older adults.
- Take full accountability for patient care and outcomes, seeking specialist consultation when needed while remaining involved in detailed patient care.
- Engage with hospitalists for patients admitted to the hospital, regardless of the hospitalist’s affiliation.
- Lead assessment, diagnosis, treatment, management, education, health promotion, care coordination, and documentation for patients with acute and complex chronic needs.
- Lead the care team (medical assistant, care facilitator, care coordinator) for office‑visiting patients.
- For non‑office patients, engage with transitional care teams, case managers, and other resources as appropriate.
- Actively manage the center, cover for other providers, and participate in recruiting patients and additional providers.
- Collaborate with Advanced Practice Practitioners, overseeing their actions while employed at the company.
- Perform other duties as assigned and modified by management.
- Availability and accessibility: open for walk‑in visits and timely response to calls and messages.
- Service orientation: provide care that the PCP would give to themselves or a family member.
- Evidence‑based medicine: stay updated on clinical, nutritional, and lifestyle interventions.
- Physician leadership: continuously develop leadership skills for patients, team, center, and company.
- Commitment to quality: collaborate with care teams to achieve high‑quality outcomes and measures.
- Influence: competently and compassionately influence patients, teams, and self to achieve optimal outcomes.
- Self‑care: maintain physical, mental, social, and spiritual well‑being to provide the best care.
- Proficiency in Microsoft Office Suite and related productivity software.
- Willingness to travel locally; national travel for training and regional recruiting as needed.
- Fluency in English (verbal and written); additional language fluency may be required for specific centers.
- Independent judgment: make use of and exercise professional discretion.
- Flexibility to work evenings, weekends, and holidays as required.
- Availability and accessibility for patients to build trust.

- MD or DO in Internal Medicine, Family Medicine, Geriatrics, or similar specialty required.
- Must hold or obtain an active State Medical License for the workplace state.
- Board certification or eligibility is preferred; ongoing maintenance (MOC, CME) required after certification.
- Valid DEA number for schedule II‑V controlled substances.
- Basic Life Support (BLS) certification (American Heart Association or American Red Cross) required within 90 days of employment.
$221,141 - $315,915 Salary. The posted pay range represents the base hourly rate or base annual full‑time salary. Final compensation depends on experience, education, geographic location, and other factors. The position may also be eligible for bonuses or commissions.
