Our care teams travel directly to patients who reside in Assisted Living, Memory Care and Group Home communities throughout Minnesota, Wisconsin and Florida and are supported by clinical operations and administrative colleagues who work remotely or at our corporate offices in Stillwater, Minnesota, and Tampa, Florida.

Join our growing practice as a Nurse Practitioner (NP) or Physician Assistant (PA) and bring exceptional team-based care directly to patients residing at Assisted Living and Memory Care communities in and around Winter Park, FL.
Position Overview:- Provide on-site, team-oriented primary care to patients within Assisted Living and Memory Care communities
- Full-time providers care for patients Monday-Thursday; option for Fridays to be a clinical or an administrative day
- Manage a panel of 200 patients (approximately 12-16 patients per/day with 14 being the avg.)
- Ability to design and manage your schedule
- Develop and implement care plans appropriate to patients and their desires
- Clinical support from Clinical Assistants and Care Coordination through LPNs, RNs and Social Workers
- Access to diagnostics such as labs and x-rays as needed
- Collaborate with community partners within home care, hospice, and other resources as appropriate
- Graduate of an accredited Nurse Practitioner or Physician Assistant Education Program
- Board Certified/Board Prepared through AANP, ANCC, or NCCPA
- Licensed or ability to obtain licensure through the Board of Nursing or Board of Medical Practice
- Ability to obtain and /or maintain DEA certification for the State of required of work
- Proven ability to communicate effectively with strong verbal skills
- Excellent interpersonal and customer service skills
- Must have a valid and unrestricted driver's license and minimum state-required liability auto insurance; local travel is required
- 2+ years of clinical experience as a Nurse Practitioner or Physician Assistant
- Compensation Base + Quality/Productivity: $125k - 170k
