Heartland Hospice in Wisconsin Rapids is seeking a compassionate Hospice RN / RN Case Manager to lead comprehensive home-based care for patients with life-limiting illnesses. You will assess, plan, implement, and coordinate high-quality nursing care in collaboration with an interdisciplinary team.
The role requires an active RN license, CPR, and a minimum of 1 year of RN experience in hospice or related fields. A $5,000 sign-on bonus is available for eligible candidates.
