Hiring Range: $54,505.75 to $66,595.75 per year

Assess, plan, and coordinate needed support and services to children with physical, cognitive, and emotional disabilities. Service Coordinators also support the immediate family members of these children and help problem‑solve any issues that arise. The position has a significant impact on the children and families served, allowing them to receive necessary services and remain in their home and community.
Responsibilities- Conduct assessments and develop and implement Individual Service Plans for children in the Children’s Community Options Program (CCOP) and the Children’s Long‑Term Support (CLTS) Medicaid Waivers Program.
- Provide direct supportive services to children and families on caseload, including information/referral, supportive counseling, and assistance in applying for financial assistance programs.
- Conduct home visits to discuss family issues/concerns and document relevant information within CCOP and CLTS programs.
- Complete Wisconsin Children’s Long‑Term Support Functional Screens (initial, annual, updates).
- Complete paperwork other than Functional Screens, such as case notes, letters, emails, faxes, funding requests/authorizations, Gainwell Technologies/Forward Health authorizations, Individual Service Plans (ISP), outcomes, referrals, review of medical records, IEPs, parent/guardian requests, incident reports through CITR, data entry into the Forward Health Portal using Eligibility and Enrollment Streamlining (EES), high‑cost funding requests, parental fee calculations, and other state and county forms.
- Make home visits to conduct intakes, reviews, check‑ins, provide support, attend school meetings or medical appointments.
- Make telephone calls to clients, providers, state contacts, therapists, evaluators, contractors, and other professionals involved in County‑operated services.
- Create and help manage CLTS Waiver Third‑Party Administration (TPA) authorizations with the Business Office staff.
- Perform all duties in compliance with the Medicaid Home and Community‑Based Services (HCBS) Waiver Manual for the CLTS Waiver Program and Medicaid Targeted Case Management regulations.
- Manage expenditures of CCOP funds, submit timely vouchers, and work closely with the Business Office staff to complete State‑required reporting systems.
- Adhere to Wood County safety policies and perform all job tasks safely.
- Demonstrate knowledge of Wood County Core Values and serve with courtesy and respect in accordance with the Wood County Code of Conduct.
- Research items and services that clients need.
- Follow established Wood County Human Services Department policies and procedures, and comply with federal and state policies and standards.
- Participate in unit meetings and attend required training/conferences.
- Ensure proper case files are maintained in compliance with Department policy and applicable regulations.
- Monitor service costs for clients in CLTS and CCOP.
- Perform any other duties as assigned.
