• Schedule:
  
  2 weeks on / 2 weeks off, 5 days per week, 8:00am – 5:00pm with 1 hour lunch break (flexible on time); flexible on the week on/week off schedule

  Office visits, follow-up visits, procedures including spinal cord stimulation, radio frequency ablation, and injections; approximately 15 patients per shift

• Board Certified/Board Eligible:

  ACLS, BLS
• Licensure:
  Active Wisconsin license or IMLC
