Transitions At Home, is currently seeking to hire a Full-Time Care Manager to lead our Non - Medical Division in our Mount Horeb location.

What does a Transitions at Home Care Manager do?The Care Manager is responsible for enrolling new clients for services, training the caregivers, and performing services for the client as necessary to maintain the personal care and comfort of the client while in their own home. The Non‑Medical Manager will spend approximately 25% of their time treating our clients. This is performed in accordance with the plan of care under the direction and supervision of the Administrator.

Qualifications /Job Requirements:
- High School Diploma or GED Certificate
- Knowledge of Medical Terminology
- Basic Computer Skills
- Must be able to read, write and communicate in English
- A valid Wisconsin Driver's License and Car Insurance
- CNA License or 1 Year of Caregiving experience
- Must be a minimum of 18 years of age
