Job Summary:
The Tribal Aging and Disability Resource Specialist (ADRS) provides culturally responsive, objective, and unbiased information and assistance to elders, adults with disabilities, and youth with disabilities (age 17.5+) within the Red Cliff community. Services are provided regardless of income or financial status and are designed to support informed decision-making, independence, and access to long-term care and community resources. A key responsibility of this role is to ensure that Tribal members receive culturally appropriate information about aging and disability resources and are able to comfortably and effectively access long-term care programs and services.
The ADRS works in partnership with Tribal programs, the Aging and Disability Resource Center (ADRC), and the Wisconsin Department of Health Services (DHS) to ensure coordinated, efficient, and equitable access to services.
Examples of Duties
Duties and Responsibilities:
Provide objective, unbiased information and assistance to individuals and families regarding long-term care options, services, and supports.Conduct comprehensive intake and needs assessments.Provide options counseling to support informed decision-making.Assist individuals in understanding eligibility requirements and service options.Provide short-term service coordination to address immediate needs.Serve as a liaison and advocate between Tribal members and ADRC or other service providersRefer individuals to appropriate services, including Benefit Specialists, Dementia Care Specialists, and other programs.Collaborate with ADRC when services are not available through Tribal programs.Conduct outreach, education, and marketing activities to increase awareness of ADRS services.Develop and distribute culturally appropriate educational materials.Provide training and support to partner agencies regarding cultural responsiveness and Tribal systems.Assist with enrollment and disenrollment counseling for long-term care programs.Support individuals in gathering documentation for financial and functional eligibility.Participate in long-term care functional screenings and related processes.Maintain accurate and timely client records in the state-supported client tracking system.Complete 100% time and task reporting in accordance with DHS requirement.Prepare and submit required programmatic reports.Collaborate with Tribal departments, external agencies, and community partners.Support continuous quality improvement and service excellence initiatives.Provide services in a variety of settings, including client homes, Tribal offices, community locations, phone, and virtual platforms.Maintain flexibility to meet the needs and preferences of individuals served.Manage inventory and distribution of durable medical equipment (DME), including tracking, storage, and coordination with individuals in need.The above duties and responsibilities are not an all-inclusive list but rather a general representation of the duties and responsibilities associated with this position. Bachelor's degree in a health, human services, or related field OR Registered Nurse (RN) licensure in WisconsinMinimum of one (1) year of experience in health or human services working with elders and/or people with disabilitiesExperience working with Tribal communities and understanding of cultural values preferredStrong communication, organizational, and computer skillsMust have a valid driver's license, vehicle and appropriate vehicle coverage and maintain driver's eligibility as a condition of employment. Be eligible to be put on the Tribe's vehicle insurance policy.Supplemental Information
Personal Contacts:
Daily communication and updates to Elderly Director. Routine and as needed contacts with elderly program staff, clients, Aging and Disability Resource Center of the North, State DHS, IRIS, Inclusa, managed care organization, Tribal staff and community members.
Physical Requirements:
Individual must be able to do all movements associated with home-visiting and frequent travel. Safely be able to a minimum of 50 pounds.
