Physical Therapist

• Bachelor‘s, Master‘s, or Doctoral Degree in Physical Therapy from an accredited school approved by the APTA.
• Current Wisconsin Physical Therapy license required.
• Experience working with adult and geriatric populations, preferably in skilled nursing or long‑term care settings.
• Experience with documentation systems; experience with Net Health is a plus.
• BLS Certification may be required from AHA or ARC.

• Opportunities available for full‑time, part‑time, and PRN schedules.

• Provide physical therapy services to adult and geriatric residents to improve mobility, strength, balance, and functional independence.
• Develop and implement individualized treatment plans and measurable goals.
• Document care accurately and maintain compliance with facility and regulatory standards.
• Collaborate with therapy and nursing teams to coordinate patient‑centered care.
• Educate residents and families on safe techniques and strategies to maximize independence.
• Participate in team meetings and care planning processes.

• Current Wisconsin PT license.
• Strong clinical skills with a focus on older adult care.
• Effective communication and teamwork abilities.
• Flexibility, professionalism, and a positive attitude.
