Home Health Physical Therapist Assistant PTA

Position Overview

The Physical Therapy Assistant provides skilled education, treatment, and interventions under the physician's plan of care in a home health setting. They collaborate with interdisciplinary team members, communicate with patients and caregivers, and ensure safe and effective therapy services.

• Deliver direct physical therapy per therapist's guidance and physician’s orders.
• Educate patients and caregivers on treatment plans and device usage.
• Monitor and report patient conditions, reactions, and progress.
• Develop and implement treatment plans aimed at pain relief and functional restoration.
• Coordinate discharge planning and provide follow-up instructions.
• Collaborate with other disciplines and participate in in-service training.
• Maintain confidentiality, accurate documentation, and professional standards.
• Ensure timely reporting and record-keeping to support patient care continuity.

• Graduate of an accredited Physical Therapist Assistant program.
• Current, unrestricted state license as a Physical Therapy Assistant.
• Valid CPR certification.
