Title:

Medical Scribe

The Clinical Informatics Specialist (CIS or Medical Scribe) at Oak Street Health supports primary care providers with clinical documentation so they can focus on delivering exceptional care. Scribes assist providers throughout the patient care journey—planning daily huddles, observing visits in the exam room, documenting encounters in real time, and following up after visits to assist with next steps. In addition to traditional scribing, CISs serve as clinical documentation assistants, mastering our value‑based care model, chronic condition documentation, ICD‑10 and CPT coding, and using this expertise to help close care gaps.

They receive extensive on‑the‑job training in clinical workflows, value‑based medicine, preventive care, population health data streams, and team‑based care. A minimum commitment of 1–2 years is expected.

• Documenting Patient Encounters (~80%)
  
  • Observe patient visits with the provider in the exam room
  
  • Document encounters in a structured note (history, assessment, plan, exam)
  
  • Assign appropriate CPT and ICD‑10 codes
  
  • Prepare After‑Visit Summaries
  
  • Consult with providers to ensure accurate, specific documentation
• Clinical Documentation Improvement (~10%)
  
  • Request and review medical records
  
  • Leverage Oak Street’s population health tools to support documentation improvement
  
  • Prepare for and support daily huddles and documentation reviews
  
  • Consult with providers on documentation opportunities
• Administrative Support (~10%)
  
  • Place orders and referrals
  
  • Address tasks supporting the care team
  
  • Perform additional clinical documentation responsibilities as assigned
  
  • Complete other duties as assigned

• Knowledge
  • Medical terminology and common medications (required)
  • Prior clinical experience, including shadowing or volunteering (strongly preferred)
  • Prior scribe or transcription experience (preferred but not required)
• Skills
  • Advanced listening and communication (required)
  • Strong computer literacy and ability to learn new technical workflows (required)
  • Fluency in Spanish, Polish, Russian, or other community languages (required where indicated)
• Abilities
  • Adapt to new workflows and quickly learn new concepts (required)
  • Type 70+ words per minute (strongly preferred)
  • Take direction and collaborate within a patient care team (required)
  • Be a self‑starter within role scope (required)
  • Maintain excellent attendance, including in‑person clinic presence (required)
  • Commit to at least 1 year (2+ years ideal) (required)
  • Work approx. 40–45 hours per week during clinic hours with predictable breaks (required)
  • Comply with hospital and Oak Street Health policies, including HIPAA (required)

Pay Range: $17.00 – $28.46 per hour (typical). The actual base salary depends on experience, education, geography, and other factors. The position is eligible for a CVS Health bonus, commission, or short‑term incentive program in addition to the base pay.
