Social Services Designee

• We are looking for a CNA that wants to grow in a Social Services role
• Ideal for someone working on a Social Work Degree or with Social Work experience
• 32 Hours / week
• Can pick up CNA shifts for more hours

• Develop, plan, implement, and evaluate social service programs in accordance with state and federal guidelines
• Identify and address residents’ social, emotional, and psychological needs
• Provide service coordination for residents and families
• Support care planning and interdisciplinary collaboration
• Ensure compliance with applicable regulations and documentation standards

• Licensed Social Worker preferred, but not required
• At least one year of supervised social work experience in a healthcare setting (elderly psychosocial experience preferred)
• Working knowledge of nursing home regulations, including MDS procedures
• Familiarity with Medicare and Medicaid reimbursement and eligibility guidelines Strong written and verbal communication skills
• Positive, can-do attitude with excellent attendance
