Monday - Friday - 8:00am to 4:30pm

The Resident Care Coordinator position assists with coordinating resident care through completion of the MDS, rounding with physicians, care planning, being available to direct care staff as a resource and providing help when needed, coordinating individual programs in order to help provide comprehensive resident care and gain accurate financial reimbursement to the facility.
Position Responsibilities- Serve as the wound care coordinator.
- Coordinate resident care through completion of the MDS.
- Serve as the Restorative Program Coordinator.
- Oversee care plan meetings.
- Participate in physician rounds and assist direct care staff.
- Perform other duties as requested.
- Two-year degree in nursing required.
- Must hold and maintain Wisconsin Registered Nursing license.
- Wound care certification, RAC certification preferred.
- MDS certification a plus.
- Current CPR certification.
- 6+ months of related work experience required.
- Intermediate proficiency with computers is required.
- Exceptional clinical skills.
- Strong oral and written communication skills.
- Self-starter with excellent interpersonal communication and problem-solving skills.
