Humana is hiring a Case Manager for Taylor County to coordinate care and support for members with complex health and social needs. In this role, you will assess member needs, develop person-centered care plans, and connect individuals to medical, behavioral, and community resources. You will collaborate closely with providers, interdisciplinary teams, and families to reduce hospitalizations, close care gaps, and improve outcomes.
The Case Manager monitors progress, advocates for members, ensures benefits are used effectively, and documents all activities in line with regulatory and Humana quality standards in a member-focused, inclusive culture.
