Greenfield Rehabilitation Agency is seeking a dedicated and compassionate Physical Therapist Assistant (PTA) to join our therapy team supporting seniors in Lomira, Beaver Dam, and Markesan, WI. We have opportunities for full-time, part-time, and PRN schedules, allowing you flexibility while making a meaningful impact on residents’ mobility, independence, and quality of life.

- Assist Physical Therapists in providing patient-centered care to seniors
- Implement individualized treatment plans to improve mobility, strength, and independence
- Guide residents through exercises and activities of daily living (ADLs)
- Monitor patient progress and provide feedback to the PT
- Collaborate with interdisciplinary team members to optimize outcomes
- Educate patients, families, and staff on safe mobility practices
- Maintain accurate documentation of therapy sessions and patient progress
- Current Wisconsin PTA license
- Strong clinical skills and a passion for working with older adults
- Excellent communication, teamwork, and organizational abilities
- Flexibility, professionalism, and a positive attitude
- Graduate of an accredited school with an associate degree as a Physical Therapist Assistant.
- Certification or license as required by the State of Wisconsin to practice as a Physical Therapist Assistant.
