Physical Therapist Assistant (PTA) – Full-Time, Part-Time, or PRN

Up to a $5,000 Sign-On Bonus for Full-Time Positions

Serving Lomira, Beaver Dam, and Markesan Communities

Greenfield Rehabilitation Agency is seeking a dedicated and compassionate Physical Therapist Assistant (PTA) to join our therapy team supporting seniors in Lomira, Beaver Dam, and Markesan, WI. We have opportunities for full-time, part-time, and PRN schedules, allowing you flexibility while making a meaningful impact on residents’ mobility, independence, and quality of life.

Key Responsibilities
  • Assist Physical Therapists in providing patient-centered care to seniors
  • Implement individualized treatment plans to improve mobility, strength, and independence
  • Guide residents through exercises and activities of daily living (ADLs)
  • Monitor patient progress and provide feedback to the PT
  • Collaborate with interdisciplinary team members to optimize outcomes
  • Educate patients, families, and staff on safe mobility practices
  • Maintain accurate documentation of therapy sessions and patient progress
Qualifications
  • Current Wisconsin PTA license
  • Strong clinical skills and a passion for working with older adults
  • Excellent communication, teamwork, and organizational abilities
  • Flexibility, professionalism, and a positive attitude
  • Graduate of an accredited school with an associate degree as a Physical Therapist Assistant.
  • Certification or license as required by the State of Wisconsin to practice as a Physical Therapist Assistant.
