Direct Support Caregiver

Responsibilities

• Provide person-centered support to individuals with developmental disabilities or complex needs in home and community settings
• Assist with activities of daily living, including bathing, dressing, meals, and mobility support
• Implement individualized support and behavior plans to promote independence and skill-building
• Support safe medication reminders and follow health and safety protocols
• Document services, progress, and incidents accurately and on time
• Accompany individuals to community activities, appointments, and social outings
• Communicate effectively with families, supervisors, and clinical team members
• Maintain a safe, respectful, and inclusive environment aligned with Sevita's values
• Participate in ongoing training and professional development
