Sevita is hiring a Direct Support Caregiver to provide compassionate, person-centered care to individuals in home and community-based settings. In this role, you will support daily living activities, community integration, and individualized care plans while promoting independence, dignity, and safety. You'll collaborate with healthcare professionals, document care, and communicate effectively with families and team members. Responsibilities

  • Provide hands-on assistance with activities of daily living (ADLs), including bathing, dressing, grooming, toileting, and feeding
  • Support individuals with meal preparation, housekeeping tasks, and maintaining a safe, clean environment
  • Follow individualized care and support plans, behavioral strategies, and health protocols as directed by supervisors and clinicians
  • Observe, document, and report changes in physical, emotional, or behavioral condition to the appropriate team members
  • Assist with or administer medications in accordance with state regulations and Sevita policies (where permitted)
  • Provide safe transportation to appointments, activities, and community events as needed
  • Encourage and support community integration, social engagement, and skill-building activities tailored to individual interests and goals
  • Use de-escalation and crisis-intervention techniques to respond calmly and appropriately to challenging situations
  • Maintain accurate, timely documentation of services provided, incidents, and progress notes
  • Collaborate with nurses, therapists, supervisors, families, and guardians to support person-centered goals and outcomes

Required Skills

  • Personal care and activities of daily living (ADL) assistance
  • Basic knowledge of developmental disabilities and behavioral health needs
  • Safe patient handling and mobility assistance
  • Medication assistance and documentation (permitted by state)
  • CPR and First Aid skills
  • Crisis prevention and de-escalation techniques
  • Infection control and universal precautions
  • Accurate, timely documentation and reporting
  • Time management and reliability in scheduling
  • Clear verbal and written communication
  • Safe driving and transportation (if required by role)
  • Basic computer or electronic health record (EHR) data entry
