  • Hours: Weeknights from 4:00 PM to 7:00 PM
Job Summary

Responsible to meet the social, spiritual, and therapeutic needs of the residents through activities, programming and continued interaction with community groups.

Compensation

Status: Part-time

  • Hours: Weeknights from 4:00 PM to 7:00 PM
  • Weekends: 9:00 AM to 3:00 PM

16.00 to 18.76 depending on experience

Key Responsibilities
  • Interviews resident to complete Initial and Annual Assessments
  • Facilitates daily events and programming to meet the needs of the residents
  • Performs one on one interaction to enhance quality of life
  • Implements Plan of Care
  • Plans own programs for monthly calendar
  • Enable residents to maximize their potential in activity participation through adaption, clues, /prompts and grading.
  • Documents quarterly and annual progress notes into Matrix.
Qualifications

Required:

  • Sixteen years of age
  • Strong desire to work with the elderly
  • Must be English literate and possess strong written and verbal communication skills.
  • Ability to maintain composure in stressful situations.
  • Knowledgeable with planning, developing and implementing resident crafts.
  • Well-developed organizational skills.
  • Valid WI Driver's License
