Sanford Health in Park Falls, WI, is seeking a Diabetes Care and Education Specialist (CDCES) to join the team. You will educate patients on self-care for diabetes, collect data, and support ongoing assessment and goal setting.
Responsibilities include nutrition therapy, insulin management education, and collaboration with the multidisciplinary team to improve patient outcomes across all ages. A registered dietitian with ACEND credentials is required.
